Terms:

1. Each survey entry will qualify for a random drawing of an Amazon gift card worth $100 each month. Only entries received from that month will be included in the monthly drawing. There will be 1 winner for every month in 2019. Winners will be notified by email.
2. Only one survey per person is allowed each month.
3. Void where prohibited. It is the submitter's responsibility to follow their institution's policies and guidelines.
4. The survey is open worldwide to product end-users only.
